Your First Visit
Our front desk team will greet you and collect your completed paperwork. After a short amount of time waiting in our reception area, you’ll be called to our treatment room so that we can ask about your oral health concerns, take any necessary X-rays, and perform a comprehensive examination of your teeth along with a cleaning. One of our dentists will explain any problems that we found in your mouth and outline the recommended forms of treatment; we can then schedule follow-up visits based on what you decide is best for your smile.
Patient Forms
To make your first visit easier, we’ve included our required new patient forms in the link below. We ask that you please complete these pages and then bring them to your first appointment. This process helps you receive our excellent care sooner by reducing your time in the waiting room.

While we’re more than happy to help with insurance, we also know that not every patient has an insurance policy and that even those with insurance may not have the coverage they need for specific procedures.
